Acute Care Pharmacy Job Summary

The Clinical Pharmacist ("Pharmacist") under general supervision, supplies medications and other pharmaceuticals needed for adequate patient care according to professional standards and regulatory requirements. Educates patients and healthcare workers relative to drugs and medications usage, adverse drug reactions, side effects, contraindications, etc. according to professional standards.
Manages the day-to-day activities of the pharmacy support staff.

Essential Functions:
  • Interprets and clarifies orders for all types of medications.
    Researches and responds promptly to drug information requests.
    Monitors drug therapies for appropriateness and formulary adherence.
    Provides therapeutic drug monitoring of targeted medications as applicable.
    Monitors drug therapy for potential drug-drug, drug-food, and drug-disease interactions.
    Advises patients, family and medical staff on use of various medications including dosage, side effects, drug interactions and composition.
    Directly supervises the work of pharmacy technicians and support staff as required by the Colorado Board of Pharmacy.

  • Prepares or oversees the compounding, labeling and packaging of sterile or non-sterile medications, products and pharmaceuticals in accordance with all regulatory requirements as applicable.
    Acts as a consultant on medication-related issues and assisting in coordinating the drug utilization programs.
    Assists in the development and maintenance of policies & procedures when warranted.
    Maintains secure storage and records of all medications in accordance with manufacturer, state and federal requirements.
    Complies with established departmental and DHHA policies and procedures, quality improvement, and safety initiatives.
    Completes and meets the competency standards including education and training that is specific to the patient populations and practice site served. Keeps job knowledge current.
    Assists in the training of pharmacists, pharmacy residents, pharmacy interns, pharmacy students, and pharmacy technicians.
    Attends and participates in departmental meetings, in-services, huddles, and departmental trainings as applicable.

  • Understands patient safety goals and other quality metrics that affect area of practice.
    Reports and follows up on medication or patient safety issues as needed; works to resolve problems in a manner that will improve patient safety and team efficiency.
    Reports medication events (adverse drug reactions and medication errors) using the organizations' event reporting system.
    Collaborates with key stakeholders across the health system regarding patient care and projects.
    Adheres to and complies with and demonstrates support for the mission and values of Denver Health.
    Adheres to Denver Health and departmental standards of behavior and attendance guidelines.

Education:
  • Doctorate Degree PharmD (preferred) or BPharm Required

Work Experience:
  • 1-3 years PGY1 residency completion preferred

Licenses:
  • PHA-Licensed Pharmacist - DORA - Department of Regulatory Agencies Required
  • ACLS-Advanced Cardiac Life Support - AHA - American Heart Association
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association
  • BCPS-Board Certified Pharmacotherapy Specialist - BPS - Board of Pharmacy Specialties
  • PALS-Pediatric Advanced Life Support - AHA - American Heart Association
  • PBID-Pharmacy Based Immunization Delivery - BPS - Board of Pharmacy Specialties

Knowledge, Skills and Abilities:

  • Strong verbal and written communication skills, ability to interact with other members of the healthcare team and with pharmacy leadership.
  • Strong organizational skills.

    Thorough knowledge of the principles and practices of pharmacy.

    Thorough knowledge of pharmacy related state and federal laws.

  • Ability to maintain accurate records and prepare reports.

    Ability to demonstrate accuracy, thoroughness, and attention to detail.

    Ability to demonstrate sound problem solving/decision making skills with practical, efficient, economical, and reasonable solutions.

  • Proficiency with Microsoft Office required.

    Proficiency with electronic health record software, Epic preferred.
