Description

The primary function of the CARE Network RN is to bridge the divide between hospital care and community-based services to promote health care along the continuum of services. Duties are geared toward providing patient-centered interventions that decrease care fragmentation and improve care outcomes for high-risk, complex clients. Responsibilities are focused on care coordination with other community providers, with the goal of enhancing the client's quality of life through improved access to services, prevention of inappropriate institutionalization and provision cost-effective service planning. Goals are centered on the Triple Aim: Improving the patient experience, improving quality of care and decreasing unnecessary health care expenditures.

Required Qualifications:



  • Coursework/Training: Graduation from an accredited nursing school or Licensure by the State of California.
  • California Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association upon hire.


Preferred Qualifications:



  • Bachelor's Degree in Nursing.
  • 2 years clinical nursing experience, or
  • 1 year of experience in community case management.
