Limb Preservation Nursing Coordinator

The Limb Preservation Nursing Coordinator will support the multidisciplinary care of patients with complex vascular and podiatric conditions who are at risk for lower-extremity complications and limb loss. This position will coordinate and follow patients throughout their continuum of care, helping to ensure timely communication, scheduling, clinical follow-up, and connection to appropriate services.

The coordinator will work flexibly between the Meadowmont and Hillsborough clinics based on patient and clinic needs. This role requires a high level of clinical judgment, organization, communication, and the ability to work independently and autonomously within established clinical algorithms and workflows.

Key Responsibilities:

• Follow patients throughout their care journey, coordinating services and ensuring appropriate follow-up.
• Serve as a clinical resource and point of contact for patients with complex vascular and podiatric conditions.
• Make and receive patient telephone calls related to care coordination, follow-up, appointments, and clinical needs.
• Coordinate and facilitate patient scheduling across vascular, podiatry, wound care, and other appropriate services.
• Identify gaps or barriers in care and work proactively with the multidisciplinary team to address them.
• Participate in and help lead PALS (Patient Alignment Limb Salvage) meetings, including preparing agendas, presenting patient information, and completing accurate meeting documentation and follow-up.
• Maintain clear and timely documentation of patient care coordination, communication, and outcomes.
• Utilize established clinical algorithms to guide patient care, escalation, referrals, and follow-up, while exercising appropriate independent clinical judgment.
• Work autonomously to manage assigned patients and coordinate complex care needs.
• Collaborate closely with vascular surgery, podiatry, wound care, nursing, providers, and other interdisciplinary team members.
• Assist with developing and improving limb preservation workflows, care pathways, and processes designed to improve patient outcomes and reduce barriers to care.
• Monitor patients who may require additional intervention and ensure appropriate follow-through with the multidisciplinary team.
• Support the development and growth of the limb preservation program across Meadowmont and Hillsborough.
• Provide patient education and reinforce treatment plans, preventive care, and follow-up recommendations.
• Demonstrate strong organizational skills and the ability to prioritize multiple patients and competing clinical needs.

Position Expectations:
This position requires a nurse who is comfortable functioning independently, managing complex patient needs, communicating effectively across disciplines, and taking ownership of patient follow-through. The coordinator must be able to navigate both vascular and podiatric clinical needs and recognize when additional evaluation, intervention, or escalation is needed.

The position will require flexibility to work between the Meadowmont and Hillsborough clinic locations to support the needs of the Limb Preservation Program.

Education Requirements:
* Graduation from a state-accredited school of professional nursing
Licensure/Certification Requirements:
* Licensed to practice as a Registered Nurse in the state of North Carolina.
Professional Experience Requirements:
* Five (5) years of professional nursing experience.
