Job Function Summary: Involves the provision of integrated and accessible primary and specialty patient care services in a clinic or center environment. Care is provided by clinicians and assistants who are accountable for addressing a large majority of personal health care needs and developing a sustainable partnership with patients. Ensures optimal patient outcomes through effective leadership and / or provision of patient care delivery in healthcare clinics or centers. Promote shared decision-making at all levels to ensure a shared voice in practice, exchange of ideas, enhanced communication, and continually improved patient services. Management levels may also be responsible for clinic operations. (Note, when primary focus is clinical operations, see Ambulatory Administration job function.) Custom Scope): The Nurse Supervisor (NS) is part of a dynamic care team within the Office of Population Health (OPH) at UCSF Health. This position is a mission-driven nurse leader that will bring a leadership presence, sound judgment, and a commitment to operational excellence in advancing innovative models of care. This NS provides operational clinical oversight for the following disciplines: Registered Nurses, Advance Nurse Specialists, other Nursing positions who work in the areas of Complex Care Management, Transitions, and other Care Coordination Programs for both Adults and Pediatrics. In addition will participate in the multiple OPH Community Events. This position will serve as a clinical leader resource and liaison for all staff within the areas they report to. The position will be responsible for overall operations in addition to the achievement of outcomes. In addition, the Supervisor will ensure compliance around all compliance and regulatory requirements. Will participate in any current or new accreditation obtained by their teams. Will be required to attend training program for the Enhanced Care Management (ECM) programs. The Supervisor will be responsible for working with providers to ensure that orders and referrals are carried out in a timely manner and ensure that any medical or psychological issues identified by the frontline team are escalated and addressed. This will be done through clinical protocols established by the OPH Leadership and Medical Directors and/or department liaisons. This position will report to the Assistant Clinical Director - Nursing. Will be part of the OPH leadership team and will collaborates closely with other OPH leaders including the Vice President Population Health, Chief Population Health Officer, Medical Directors, other Managers and Director and key internal UCSFH Health and external community partners. This is an on-site leadership position requiring a four-day-per-week presence or as directed by the telework agreement. Will need to have flexibility to work across UCSF Health locations (East and West Bay).

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF.
