Job Description
General Purpose of Job: Provides comprehensive assessment, short-term counseling and referral services to CARES clients. Provides training and consultation to service existing company contracts. Essential Duties: This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Conducts comprehensive intake for clients and their household members and arrives at a diagnosis and treatment plan.
- Implement client's individualized treatment plans and when clinically appropriate conducts ongoing time limited individual, family, and group treatment sessions. Appropriate referrals are made when indicated to best meet the needs of individual clients.
- Maintains client records in compliance with department standards.
- Maintains and consults on a regular basis with contract companies to meet their individual needs.
- Develops training material and conducts training sessions for contract companies, supervisors, labor representatives, and employees.
Job Requirements
| General Requirements |
* Current LMSW licensure by State of Michigan; OR LLMSW licensure and previous behavioral health experience * BLS - preferred * Certification in Case Management - preferred |
| Work Experience |
*Minimum three years of social work experience in an acute care setting *Ability to interview, assess, organize and problem solve. Ability to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, and interdisciplinary team and community agencies to achieve desired patient outcomes. |
| Education |
* Master's Degree in Social Work * Recipient Rights training with 30 days of hire and annually thereafter. * Corporate Compliance training with 30 days of hire and annually thereafter. |
| Specialized Knowledge and Skills |
*Maintains an environment that supports and encourages high performance and reliability through caregiver engagement, teamwork, and customer service ensuring safe quality patient-centered care. *Possess excellent verbal and written communication skills. *Promotes individual professional growth and development by meeting the requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization. *Ability to work independently and exercise sound judgement in interactions with physicians, payors, patients and their families. *Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. *Experience in acute care case management, discharge planning, utilization management, performance improvement and managed care reimbursement preferred |
