POSITION SUMMARY:

Provides quality psychosocial care to our patient's and coordinate care with medical team. Through the coordination of care, the primary purpose is to conduct a thorough assessment of patients' needs, understanding of the medical plan of care, coping abilities, and barriers to care then implement interventions and/or provide resources. Serves as an integral member of the health care team delivering psychosocial care in accordance with JHACH policies and procedures and state and federal regulations.

KEY ACCOUNTABILITIES:



  1. Conducts psychosocial assessments to identify and respond to patient and families' social and emotional needs which impact the patient's medical treatment and outcome
  2. Conducts a suicide risk assessment to determine level of risk and then make appropriate safety recommendations to the medical team
  3. Makes necessary referrals to multi-disciplinary team members and links patients and families to community resources to assist in coordination of care and to reduce psychosocial stress
  4. Provides therapeutic interventions such as crisis intervention, protective services, grief/supportive counseling, problem-solving, strengths perspective, utilization of healthy coping strategies, and psychoeducation
  5. Collaborates with medical team to develop a plan of care and identifies psychosocial barriers which interfere with, or prevent, discharge
  6. Maintains timely electronic psychosocial documentation, data collection, and proper record keeping for patients
  7. Communicates with team members regarding the patient's needs and interventions utilized as well as attends team meetings, huddles, and/or rounds



QUALIFICATIONS:



  • Master's Degree in Social Work
  • Completion of JHACH's intern program to meet the qualifications of the Social Worker career requirements OR have successfully completed a comparable intern program in an external healthcare environment
  • Excellent listening and oral/written communication skills
  • Ability to document effectively in the electronic medical record


Salary Range: Minimum 27.85/hour - Maximum 44.57/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
