JOB DESCRIPTION:

SUMMARY OF POSITION

Serves as the liaison for coordination of accessing care between Centerstone Health practice and Centerstone Indiana. Primary responsible for preforming administrative task as it relates to requesting and obtaining medical records for patients who have a scheduled evaluation with a Psychiatric provider. Responsible for assembling all gathered information for each provider's review. Assists the Call Center Manager with the review and approval process of Psychiatric referrals received for Centerstone Health and Centerstone of Indiana. Assists the Call Center Manager with the scheduling and review of Psychiatric evaluation appointments with Centerstone Health Services. The Referral coordinator will work to integrate the operational services with Centerstone's mission to help improve process and performances for the delivery of high-quality services.

ESSENTIAL DUTIES & RESPONSIBILITIES

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Skilled in active listening.
  • Proficiency in MS Office (Word, Excel).
  • An effective team player with strong inter-personal skills.
  • Demonstrated ability to work and make decisions in a fast-paced environment.

QUALIFICATIONS

Education Level

HS Diploma or GED required.

Years of Experience

Minimum of one (1) year of related experience in health care service.
