Responsible for providing comprehensive education and support to patients with diabetes, promoting self-management skills, and enhancing health outcomes. This role involves working collaboratively with healthcare teams to develop individualized care plans and facilitate diabetes management programs.
Does this position require Patient Care? Yes Essential Functions: -Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management. -Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, and blood glucose monitoring. -Facilitate group education classes on diabetes management, prevention, and lifestyle modifications. -Create educational materials and resources to enhance patient understanding and engagement. -Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes. -Encourage patient adherence to treatment plans and empower them to actively participate in their health. Education Bachelor's Degree Related Field of Study preferred Master's Degree Related Field of Study preferred The Diabetes Educator will play a key role in empowering patients through education, clinical guidance, and ongoing support, working closely with providers and caregivers to improve longterm health outcomes.
