UNC Health Medical Group is looking for a Per Diem Advanced Practice Provider (NP/PA) to join the growing UNC Lenoir's Transitions of Care Clinic as a virtual APP. If this is the position for you, please apply!

Responsibilities:

• Provide remote patient management and care, collaborating with healthcare providers.
• Utilize electronic medical records (EMR) systems to facilitate communication and care delivery.
• Manage patient communication, including scheduling appointments and sending reminders.
• Facilitate video consultations and ensure timely responses to patient inquiries.

1 or 2 days a week Hours (Per Diem)

Hours:8:00 a.m. - 4:00 p.m.

EMR: EPIC

Qualifications:

• Graduation from an accredited NP or PA school
• One or more years of full panel primary care experienced required
• North Carolina licensed
• BLS & ACLS certified
