Job Title: Communicable Disease Investigator Location: Louisville KY, Duration: 1+ Year with possible extension Pay Rate Range: $20-$25/Hr Role & responsibilities: Provide case management and counseling to individuals diagnosed with STDs and/or HIV. Connect individuals to testing, treatment, and supportive resources. Conduct field investigations and outreach to notify individuals who may have been exposed. Collaborate with healthcare providers to support quality care and follow-up. Provide education and prevention counseling to help reduce reinfection. Qualifications: Bachelor's degree (preferred in social work, public health, or a related field). Experience in case management, social services, or public health.
