Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. It is expected that the employee demonstrate behavior consistent with our core values:Integrity, Accountability, Best Practices, Compassion and Synergy. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities.The Hospice LVN provides intermittent visits to the terminally ill patient and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing and psychosocial support to provide appropriate care as the patient transitions to the endof life

Essential Functions and Responsibilities of the Job

1. Assesses present and potential needs of patient and family
2. Review Plan of Care with patient, family, physician, IDT and review and evaluate
3. Review realistic goals of care to ensure continuity and quality of care
4. Complete all documentation in a concise, timely manner per agency standards
5. Manage symptoms and teach patient/family care management
6. Ability to be at work and on time
7. Ability to follow company policies, procedures and directives.
8. Ability to interact in a positive and constructive manner.
9. Ability to prioritize and multitask.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
