Location: Calmont Operations BuildingDepartment: AdministrationShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: The AVP, Care Management, Cook Children's Health Plan (CCHP), oversees the strategic program development, integration, direction, and leadership of CCHP's Member-facing clinical teams. This leader assures that the Member-facing clinical programs comply with contractual, regulatory and accreditation standards. The AVP, Care Management is charged with being a partner to local providers, Community Based Organizations, and Cook Children's Health Care System (CCHCS) in initiatives to improve healthcare quality, decrease preventable morbidity and mortality, and enhance the health of the population served.Additional Information: The AVP, Care Management, Cook Children's Health Plan (CCHP), oversees the strategic program development, integration, direction, and leadership of CCHP's Member-facing clinical teams.
