Responsibilities
1. Patient Care 1.1 Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection 1 2. Contribute to the development and implementation of educational programs for patients, families, and staff 1 3. Advise patients as to long-term complications 1 4. Assist patients in monitoring blood glucose for the purpose of medication control and adjustment of both insulin and noninsulin-dependent diabetics 1 5. Assess, monitor, implement, and evaluate treatment plans for patients at various stages of the disease with a focus on prevention of complications and management of stable diabetes. 1 6. Provide ongoing assessment and recommendations to the treatment team and patient/family/support systems for adjustments in the plan of care 1 7. Document assessments, interventions, and plans including recommendations and goals, progress notes, and discharge summaries 1 8. Collaborate with individual patients and groups on setting goals and objectives within the diabetes care plan and per patients goals. Uses motivational interviewing techniques to set goals with patients to promote glycemic control and overall health. 1 9. Leads group educational sessions. 2. Education/Training 2 1. Contribute to the development of curriculum and content for clinical staff related to diabetes 2 2. Facilitates training 2 3.Actively participates in educational and sharing opportunities outside of the organization to develop expertise 2 4. Propose changes to curriculum that would improve care delivered to our patients 2 5. Provide clinical resource and consultation to staff and clinicians 3. Patient and Family-Centered Care 3 1. Supports the organizations commitment to patient and family-centered care 3 2. Developing and maintaining respectful, cooperative working relationships to contribute to delivery of optimal care to our patients and families
4. Documentation 4 1. Documents plan of care to address the patient's medical condition. 4 2. Maintains continuity of patient care assignment 4 3. Completes all Patient Health Information (PHI) forms. 4 4. Maintains patient medical records. 5. Other Requirements 5 1. Ability to work in a collaborative practice model and willingness to assist with patient care when required 5 2. Capability to adapt quickly to a dynamic work environment 5 3. Strong health assessment/clinical skills 5 4. High degree of accuracy and attention to detail 5 5. Participates in performance improvement and continues quality improvement activities 6. May assist and/or perform chaperone duties for patient medical procedures as needed
Qualifications
EDUCATION (number of years and type required to perform the position duties): Bachelors in Nursing preferred RN Licensure required Certified as a Certified Diabetes Care and Education Specialist (CDCES) or eligible to sit for examination within 12 months EXPERIENCE (number of years and type required to meet an acceptable level of performance): At least 2 years of experience in a healthcare setting, preferably in diabetes prevention or health education program SPECIAL SKILLS: Current skills and knowledge in diabetes, including anatomy and pathophysiology of diabetes and associated complications. Ability to design/implement/evaluate health programs policies and procedures. Excellent leadership , organizational and communication, and problem-solving skills. ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or other areas of accountability): Will be accountable for following policies and procedures and maintaining relevant knowledge, skills, and leadership competence through continuing education. Responsible for engaging patients and families and providing education on diabetes. Position Overview
Provides state-of-the-art diabetes education in the outpatient setting for persons with diabetes and their families /care partners in order to optimize diabetes management. Participates in the development of educational materials focusing on disease process, medication management, diet, prevention and care complications, and blood glucose monitoring. Acts as a health consultant by providing information on the management of diabetes for all patients of ages from infancy to geriatrics. Collaborates with treating physician in patients care.
