Virginia Beach Psychiatric Center (VBPC) is a 100-bed freestanding psychiatric hospital located near the beautiful resort area of Virginia Beach, Virginia; a short 10-minute drive to the oceanfront. VBPC offers acute psychiatric and substance abuse services for adults 18 years and older. The hospital is accessible 24 hours a day for admissions and includes mental health assessments and an excellent referral network. Treatment programs and specialty services at VBPC include substance abuse, dual diagnosis and partial hospitalization. Virginia Beach Psychiatric Center currently has an opening for Full Time Step-Down Care Navigator. Candidates must be available to work occassionally on weekends and holidays. The stepdown navigator is responsible for meeting with all eligible patients to discuss the benefits of outpatient services as part of their continuum of care treatment. The step-down navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes. He/She will have a thorough understanding of the step-down playbook, implementing components of the playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Qualifications High School Graduate/GED required Bachelor's degree preferred 1-3 years' experience in psychiatric/mental health field required Must possess knowledge of the disease concept of addiction; chemical dependency treatment processes; 12-step recovery programs; dual diagnosis; group dynamics; supervisory skills; and advanced oral and written communication skills Must be knowledgeable of and abide by HIPAA regulations regarding protected health information as defined by HIPAA and PSI. Performs other duties as assigned/required by this position.
