* Conduct Biannual visits and document findings

* Follow up calls to members

Education/Certification Required Licensed Practical Nurse or Licensed Vocational Nurse Preferred:

Licensure Required: Licensed Practical Nurse or Licensed Vocational Nurse Preferred:

* Years of experience required

* Performance indicators Must haves: Experience in completing assessments and case management preferred. Familiar with Outlook (Microsoft, Excel, Outlook emails)

Nice to haves: Previous MCO experience
