$3,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.

The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, and Assistive Living Facilities. The PT assumes responsibility for participants treated by the PTA.

Required Qualifications:



  • Bachelor's Degree Physical Therapy.
  • Within 30 days of hire: National Provider BLS - American Heart Association.
  • Upon hire: Washington Physical Therapist License.


Preferred Qualifications:



  • Master's Degree Physical Therapy. Or
  • D.P.T. Physical Therapy.
  • 1 year Working with a geriatric population.
