Per Diem Clinical Review Manager (CRM) - Dual Certification Required

$40/hour | Remote

Activa Home Health (an AccordCare Company), a licensed Medicare-certified home health agency, is seeking an experienced Per Diem Clinical Review Manager (CRM) to support our patients and clinicians. Requirements:


  • Dual certification in OASIS and diagnosis coding (required)



  • Proven experience in clinical review and home health documentation



  • Ability to assist the team with overflow charts, especially during month-end and PTO coverage



  • Strong skills in reviewing and completing 30-day summaries



  • Experience with face-to-face review with coding and QA teams



  • Knowledge of creating 485 forms for SOCs and updating recertification OASIS forms



  • Ability to review post-facility discharges and create post-hospital orders



  • Experience in reviewing and updating coding for OASIS follow-ups (SCIC)



  • Strong communication and collaboration skills with field clinicians regarding OASIS documentation updates



  • Ability to provide education and follow-up as needed to ensure accurate and compliant documentation




Responsibilities:


  • Support team with overflow charts and coverage needs



  • Conduct face-to-face reviews with coding and QA teams



  • Create and update 485 forms for SOCs and recertifications



  • Review post-facility discharges and create post-hospital orders



  • Update coding for OASIS follow-ups (SCIC)



  • Collaborate with field clinicians on documentation, provide education, and ensure follow-up
