About Corewell West Cardiovascular Medicine Cardiovascular Medicine is the largest outpatient care center for all cardiovascular disorders. We have 50+ physicians and 50+ advanced practice providers that see patients mainly in the 13-county service area. Our providers cover both outpatient and inpatient care inclusive of a variety of invasive and non-invasive procedures, cardiovascular testing and monitoring and multi-disciplinary care. The tasks include scheduling patients for comprehensive medication reviews (CMRs) and/or chronic disease management from provider referrals or other registries and transcribing the pharmacist's encounters into the OutcomesMTM platform to assist with capturing quality measures and clinic incentives. This role also involves working with insurance companies to review coverage options, completes prior authorizations, and assists with patient assistance programs. Will work collaboratively with pharmacy leaders and other departments to monitor and support quality and access initiatives and to improve operational efficiencies.
