Cardiovascular Medicine is the largest outpatient care center for all cardiovascular disorders. We have 50+ physicians and 50+ advanced practice providers that see patients mainly in the 13-county service area. Scope of Work Responsible for supporting multiple pharmacists. The tasks include scheduling patients for comprehensive medication reviews (CMRs) and/or chronic disease management from provider referrals or other registries and transcribing the pharmacist's encounters into the OutcomesMTM platform to assist with capturing quality measures and clinic incentives. This role also involves working with insurance companies to review coverage options, completes prior authorizations, and assists with patient assistance programs. Will work collaboratively with pharmacy leaders and other departments to monitor and support quality and access initiatives and to improve operational efficiencies.
  • Identify and schedule Comprehensive Medication Reviews (CMR's) and/or Chronic Disease Management appointments for pharmacists from provider referrals, quality reports, and other available registries.
  • Transcribe patient charts into OutcomesMTM and manage claims within the platform.
  • Assist pharmacists and patients with insurance barriers, Prior Authorizations, and parts of the patient assistance process for their patients.
  • Manage multiple schedules and be a support for our patients and pharmacists.
  • Organize and track reports and data for multiple payer programs.
  • Communicate positively and effectively with patients, colleagues and clinic staff.
  • Maintains all requirements for CPhT certifications. Completes all Spectrum Health assigned competencies by assigned due dates.
  • Perform special projects, other duties, and participate in workgroups or committees as assigned.
Qualifications
  • Required High School Diploma or equivalent
  • Preferred Associate's Degree
  • 1 year of relevant experience Ambulatory or retail pharmacy Preferred
  • 1 year of relevant experience Electronic Health Record and MTM platform Preferred
  • LIC-Pharmacy Technician - STATE_MI State of Michigan Upon Hire required Or
  • LIC-Pharmacist Intern - STATE_MI State of Michigan Upon Hire required
  • CRT-Pharmacy Technician, Certified (CPHT) - PTCB Pharmacy Technician Certification Board 90 Days required Or
  • CRT-Exam for the Certification of Pharmacy Technicians (ExCPT) - NHA National Healthcareer Association 90 Days required
  • Ideal candidate will possess Full Pharmacy Technician License and National Certification, such as PTCB Certification or ExCPT
  • Ideal candidate will possess Prior Authorization and Patient Assistance experience
  • Community Pharmacy experience would be a great plus
  • This is an On-Site, Monday to Friday position, 8 a.m. to 4:30 p.m.
  • No Holidays, No Weekends.
  • There may be a possibility for Hybrid Work after several months of training.
  • This position will support both Advanced Heart Failure and Cardiovascular Medicine and the split will be 50/50.
