Summary Job Description: The Behavioral Health Navigator II is a key component to our Bridge to MAT program. It requires flexibility as we attempt to assist patients in the ER, Behavioral Health inpatient units, Medical floor inpatients and those in outpatient settings seeking help for Opioid and/or Stimulant use disorders. A key focus for our bridge program is to connect patients with Medication Assisted Treatment when that is appropriate. We also assist them in connecting with other forms of treatment that may include residential treatment, intensive outpatient treatment or crisis stabilization. The Navigator II position requires overseeing aspects of the grant that includes gathering of data that includes patient assessments and follow-up assessments. Completing financial assistance forms and distributing gas cards to those in need of assistance. The Navigator also plays a role with current inpatient detox patients and those attending our IOP program. It is primarily in a peer support capacity for the inpatient and IOP. Minimum of a bachelor's degree in social work, human services, counseling or related field and two years of experience in working in the field of behavioral health preferred. Must be trained in CPI or its equivalent and CPR.
