What You'll Do



  • Provide field-based mental health and case management services in homes, hospitals, and community settings
  • Build strong, trusting relationships to support members' recovery goals
  • Collaborate with community partners to connect members with housing, benefits, employment, and medical care
  • Conduct psychosocial assessments, service plans, and crisis interventions
  • Offer cognitive-behavioral and skill-based interventions that promote stability and independence
  • Participate in 24-hour on-call rotation (shared among clinical team)
  • Provide transportation for members served as needed (mileage reimbursed weekly at IRS rate)

Qualifications

Required:



  • Master's degree in psychology, social work, counseling, or related behavioral science field
  • Registered as a Qualified Mental Health Professional (QMHP) in the State of Oregon
  • Current CPR, CPI, and First Aid certification (within 60 days of hire)
  • Valid driver's license, auto insurance, and willingness to use personal vehicle (mileage reimbursed)
  • At least 18 years old


Preferred:



  • Bilingual (Spanish or other language)
  • Experience with housing, dual diagnosis, or vocational rehabilitation
  • Familiarity with recovery-oriented care and community-based behavioral health services


Compensation

Expected starting range: $26.46 - $32.69/hour
Pay is based on experience and geographic differential.

About Telecare Oregon ACT

Telecare's Oregon Assertive Community Treatment (TACT) program supports up to 150 adults (ages 18-59) living with serious mental illness. Our interdisciplinary team provides wraparound, field-based care - from medication management and crisis support to housing and employment services - helping members achieve stability and community connection.
