Social Worker

We are seeking a licensed social worker (LMSW or LCSW) to provide clinic services to youth in a school-based setting. The position involves working Monday through Friday from 8 am to 4:30 pm.

• Maintain an average clinic caseload of approximately 25-30 students ranging from kindergarten to 12th grade.
• Participate in all admissions with the child, family, and telehealth MD through NYU.
• Work collaboratively with our NYU telehealth fellows on each youth's treatment needs.
• Provide Individual and Family Therapy and crisis intervention as needed.
• Engage in collateral involvement with Saugerties Central School District and any outside agencies involved in the youth's care.
• Function as part of an interdisciplinary treatment team.
• Participate in weekly Treatment Team meetings.
• Advocate for youth and families to obtain community-based linkages with additional supportive services that meet their needs.
• Maintain timely medical record documentation.
• Facilitate thorough discharge planning and complete discharge follow-up.
• Collaborate with school liaisons within Saugerties to facilitate a streamlined and efficient referral and screening process.
