This includes: Making the initial nursing evaluation and creating an appropriate and patient-specific plan of care Identifying the patient's/family's physical, psychosocial, and environmental needs and tailoring each care plan appropriately Completing recertifications or resuming care for eligible patients Initiating and coordinating the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition Documenting problems, appropriate goals, interventions, and patient/family response to skilled nursing care Collaborating with the patient/family, attending physician, and other members of the IDG in providing patient and family care Experience Requirements: One year of clinical experience preferred. License Requirements: Current Wisconsin RN licensure Current driver's license, valid vehicle insurance, and access to a dependable vehicle Position Details: Location: Fond du Lac and Dodge Counties Schedule: Full-Time, 1st shift Monday-Friday.
