Summary:
The Registered Nurse - Patient Navigator - Cardiac Surgery role supports safe patient and family centered care by meeting the specialized needs of patients in the assigned service line as they progress through a wide variety of health care settings. The position is responsible for facilitating care and surgical services to enhance quality, cost-effective clinical outcomes, to both meet patients' healthcare needs while also decreasing fragmentation and duplication of care.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Graduate from an accredited college with a Bachelor's Degree in Nursing required. Master's Degree preferred.
  2. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required.
  3. Experience: Experience working with patients within the service line preferred, in inpatient, outpatient or community-based care settings.
  4. Additional Skills/Requirements Required: N/A
