Description
The Medical Social Worker is an integral part of the interdisciplinary care team, providing comprehensive psychosocial support to patients, families, and caregivers. This role is essential in addressing psychosocial factors, mental health concerns, caregiver support, and access to community resources, directly impacting the effective treatment of medical conditions and the overall well-being of patients. The Medical Social Worker collaborates closely with clinical teams to ensure holistic, patient-centered care. Position: Medical Social Worker for Population Health Wage Range: $65,000-$75,000 annually, depending on experience Service Area: Minneapolis/St. Paul, metro area. Plus, mileage reimbursement of .70 cents per mile. Hours: Monday through Friday, 8am-5pm.
