Job Description
General Purpose of Job: This role primarily oversees daily clinical care, staffing, and operations of hospice services in the home and/or hospice house environments in collaboration with the Manager, Medical Director, and Director to ensure safe, quality care and quality patient experience. Also helps ensure appropriate documentation, competence of team members, and compliance with regulatory requirements. Provides leadership on-call coverage on a rotating basis as well as direct patient care and clinical on-call coverage intermittently when needed due to gaps in staffing and/or high patient acuity. The Clinical Supervisor will also work with the Hospice Quality and Education Supervisor to support quality and education initiatives and be cross trained to cover key functions of the Hospice Quality and Education Supervisor role such as submission of data for mandatory reporting. |
Essential Duties: This job description is intended to cover the minimum essential duties assigned on a regular basis. Caregivers may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- In coordination with the Manager, provides supervision, gives feedback on performance and offers coaching and teaching
- Responsible for scheduling, staffing and payroll of clinical team members
- Works closely with the Clinical Manager by responding to concerns, clinical issues and learning/growth opportunities.
- In collaboration with Clinical Managers, supervisor helps select and educate team members for positions with Home Hospice and Hospice House. Educates, supervises, counsels and evaluates all caregivers on orientation as requested by Clinical Manager/ Director. Provides team members with support, encouragement and motivation to perform job duties in an optimal manner. Oversees completion of orientation documentation.
- Assists in development and helps maintain department policies and assists in development and maintenance of policies related to general department operations, discharge planning and patient care.
- Participates in department Performance Improvement, Infection Prevention, and team member engagement initiatives. Identifies opportunities for improvement. Plans and implements strategies to improve department operations and patient outcomes in conjunction with Sparrow Quality Department, Clinical Managers and Director.
- Provides education to department staff, physicians, hospital departments and others related to department operations and responsibilities.
- Assists in the development of department goals and objectives. Helps implement plans to achieve goals. Participates in efforts to support and advance the mission, vision, strategies and goals of the Health System.
- Monitors customer satisfaction, performance improvement and corporate compliance indicators, and key indicators of effectiveness and efficiency to ensure optimal performance.
- Maintains proficiency in computerized documentation practices and can mentor others as they learn software documentation.
Job Requirements
General Requirements |
* Current Michigan Registered Nursing License. * BCLS through American Heart Association. * Active, valid State of Michigan Driver's License. * Proof of active car insurance. * Nursing Certification is preferred |
Work Experience |
* Minimum of three years direct nursing care experience. * Hospice experience is preferred * Leadership experience is preferred * Experience with Epic, Kronos, Press Ganey, RL Solutions and / or CMS Hospice Compare software/ websites is preferred. |
Education |
* Bachelors in Nursing (BSN or BAN) |
Specialized Knowledge and Skills |
* Critical thinking and problem-solving skills * Excellent documentation and communication skills * Experience with EMRs and Microsoft Office * Ability to handle multiple tasks concurrently * Ability to quickly adapt to change. |
