Description MaineHealth Behavioral HealthProfessional - Clinical For a limited time MaineHealth is offering up to$10,000 Sign on bonus for all eligible Clinical Case Managers! Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 3 months separation from employment. Bonuses may be prorated based on schedule. ~ 40 Hours/week, Full-Time, Day shift ~ Maine state MHRT-C certification is required if not clinically licensed, Licensed Social Worker (LSW) preferred Summary The Clinical Case Manager I - ACT role provides mental health rehabilitation services, supporting adults and families to reduce hospitalization and crisis episodes. This role delivers essential services, primarily in the clients living situation, including crisis intervention, case management, individual coaching, monitoring for medication adherence and skills training. MaineHealth Behavioral Health provides assertive community treatment (ACT) services for people with significant behavioral health needs. Our multidisciplinary team includes psychiatric providers, registered nurses, case managers, therapists and an employment specialist. The ACT program is based on national standards and is intended to prevent psychiatric hospitalization and criminal justice involvement.The goal is to improve patient functioning in the least restrictive setting, while strengthening family, work, school and community ties.
