Appendix A Profiling Physician We are seeking a highly skilled and passionate Profiling Physician to join our team. As a Profiling Physician, you will be responsible to provide services in support of Individual Medical Readiness to the Army Reserve Medical Management Center (AR-MMC). This role shall provide a wide range of services to Soldiers found to have medical issues or concerns that deem them medically not ready (MNR) for mobilization/training and require referral management for care/treatment or for appropriate medical board. The Profiling Provider is responsible for transcribing and developing physical profiles in response to request from intake sources (Command, Soldier, Reserve Health Readiness Program (RHRP) or AR-MMC Intake Team) utilizing the supporting documentation with the rationale for the Soldier's MNR status. The Profiling Provider is responsible for communicating with the assigned Permanent Case Reviewer (PCR) or Temporary Case Reviewer (TCR) details regarding the Soldier's case, as well as any communication with the Soldier's Command at the inception of the profile. The Profiling Provider will communicate any resourcing needs with the assigned Permanent Case Reviewer or Temporary Case Reviewer for further resourcing with the USAR Psychological Health Program. The Profiling Provider is expected to demonstrate familiarity and proficiency with the medical standards of service and the Disability Evaluation System process as defined in AR 40-501, AR 40-66, AR 635-40 and DA PAM 635-40. ---- Responsibilities: The Profiling Physician will:
- Provide guidance and assistance to Soldiers who require referrals for medical treatment or care and assist Soldiers with Individual Medical Readiness (IMR) support to include identification and tracking of Soldiers who are not medically ready.
- Collect and review performance data and work with the Army Reserve (AR) Surgeon's office to tailor processes to meet specific USAR wounded, ill and injured Warriors, and coordinate with USAR Recovery Care Coordinators (RCCs) for non-medical case management and family needs, to deliver the highest possible quality of care support.
- Work in collaboration with the Regional Support Command (RSC) Surgeons to facilitate identification of the medically not ready (MNR) population, ensure assessment of the MNR population and will coordinate with unit leaders and available medical options to determine what the MNR Soldiers need.
- Assist Soldiers in managing referrals for care and in becoming medically ready or begin medical boarding packet building.
- Provide guidance and assistance to Soldiers who require referrals for medical treatment or care and assist soldiers with individual medical readiness (IMR) support to include identification and tracking of soldiers who are not medically ready.
- Coordinate with Soldier to assess and develop plan of care or treatment of conditions that have or will result in Soldier's medically not ready status.
- Manage MNR Soldier's plan of care from receipt of case referral or notification through case closure, as required.
- Perform intake interview with Soldier and establish communication with command.
- Ensure Soldiers understand his/her responsibilities for making and keeping appointments and provide the Physician with medical documentation.
- Responsible for educating the Soldier about their privacy rights and how their information may be used and obtains consent for release of medical information.
- Secure documents and assist in entry of documents required to complete a Line of Duty (LOD) packet for submission.
- Assists MNR Soldier in arranging health care with Military Treatment Facilities, Veteran's Administration (VA), TRICARE network providers and non-network providers.
- Provide a biweekly case report to the RSC, MMA Surgeon or designated representative. The report should include number of cases opened, closed, referrals made, important unresolved issues and the number of cases identified needing a Medical Evaluation Board (MEB).
- Provide entry into the tracking system (currently MNR tracker).
- Educate Soldiers on resources available to improve medical and dental readiness.
- Develop and implement a plan of care and improvement with collaboration with the RSC Surgeon or designated representative.
- Establish and maintain a usable schedule, track and report work hours and expenses weekly, evaluate all deliverables to ensure high quality, ensure projects meet internal and external expectations, customize reports required by client, use expert judgment in all areas of this project, be prepared to discuss project quality, team satisfaction and success and metrics weekly.
- Continually seek improvement to increase client satisfaction, manage day to day operations, and develop lasting relationships with client personnel.
- Work collaboratively with the Case Managers and designated military personnel.
- Performs other duties as assigned.
Requirements: Includes, but not limited to: Primary Care and Specialty Care Physicians
- Degree: Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.).
- Education: Graduation from a medical school in the United States, Canada, or approved by the Liaison Committee on Medical Education of the American Medical Association (AMA), graduation from a college of osteopathy approved by the American Osteopathic Association (AOA), or graduation from a medical school with evidence of Educational Commission for Foreign Medical Graduates (ECFMG) certification.
- Certification: The DoD only recognizes certifications issued by boards that are members of the American Board of Medical Specialties (ABMS) or the AOA Bureau of Osteopathic Specialists.
- Internship/Residency/Fellowship: Completion of an accredited residency approved by the Accreditation Council for Graduate Medical Education (ACGME).
- Experience: As required to meet clinical competency requirements specified in the credentialing instructions.
- Licensure: Current, full, active, and unrestricted license to practice medicine for the duration of the contract.
- Foreign Medical Graduates. Certification by the Educational Commission for Foreign Medical Graduates (ECFMG) for physicians. Contract workers who are graduates of foreign medical schools will possess both a U.S. medical license and certification by ECFMG for health care physicians as defined in 29 CFR 825.125. Residency institutions must have been ACGME accredited during the time the member was enrolled. ECFMG is required for graduates of medical schools located outside the U.S., Puerto Rico, and Canada. As of 2017 a provider who possesses Fifth Pathway is no longer acceptable.
- All contract healthcare providers shall be required to meet the Continuing Medical Education (CME) requirements as required by their license.
Knowledge, Skills, and Abilities: Computer skills to enable them to develop working knowledge of Medical Operational Data System (MODS), Medical Protection System (MEDPROS), Medical Readiness Portal, Medical Evaluation Board (MEB) Prep, Joint Legacy Viewer (JLV), Electronic Medical Management Processing System (eMMPS), Integrated Personnel and Pay System (IPPS-A), Personnel Electronic Records Management System (iPERMS), Commander's Strength Management Module (CSMM), Medical Electronic Data (for) Care History And Readiness Tracking (MEDCHART), and Reserve Health Readiness Program (RHRP) as required for performance of services. You will receive training to use these systems.
- Must be proficient in Microsoft Word, Excel, PowerPoint, Outlook, and Adobe PDF.
- Ability to communicate effectively orally and in writing.
- Additional requirements include:
- U.S. Citizenship.
- Possess communication skills, problem solving skills, solid background in general medicine.
- Strong organizational skills and ability to work independently are also required.
- Ability to work collaboratively with the Case Management Team and designated military personnel.
WORK SCHEDULE & LOCATION
- Work hours are typically 0800 - 1700, Monday through Friday, excluding U.S. Federal Holidays.
- The Government may permit telecommuting (teleworking) when determined to be in the best interest of the Government in meeting work requirements.
