Responsibilities

Employment of Physician. Subject to the terms and conditions of this Agreement, Group employs Physician on a part-time basis to render professional medical services to Group's patients in Physician's Specialties, and Physician accepts such employment with Group. Physician's employment is conditioned upon Physician's (i) accurate completion, and Group's acceptance, of all pre-employment requirements including, but not limited to, criminal background checks, Group's disclosure questionnaire and such other requirements consistent with Group's policies and procedures ("Pre-Employment Documentation"), (ii) maintenance of a current license to practice medicine in the State; (iii) obtaining full medical staff privileges at the Hospitals (as defined in Section 1.2 below); (iv) maintenance of a DEA certification.


Qualifications

  • Professional Duties. Physician shall provide professional medical services in the Specialties to Group's patients at 615 E. Oklahoma Avenue, Enid, Oklahoma 73701, and such other locations as Group directs from time to time (collectively, the "Offices"), and St. Mary's Regional Medical Center, and such other hospital as Group directs from time to time (collectively, the "Hospitals") (the Offices and Hospitals are collectively, the ''Facilities"). Physician shall devote his/her full professional time, skill and attention to the performance of his/her duties hereunder. As a part-time employee, Physician is expected to work a schedule of two weeks on, two weeks off, in the performance of the duties hereunder (excluding call coverage). Physician's duties are more fully set out in Exhibit A, attached and incorporated herein by reference.
