Responsibilities

Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for a Step-Down Navigator.

The Step-Down Navigator is responsible for managing the workflow of the step-down process, ensure eligible patients are identified and engaged in PHP discussion as a next step in treatment prior to inpatient discharge. Is responsible for data collection, communication and report generation to the involved departments and facility CEO.

Essential Job Responsibilities:

  • Meet with eligible patient to discuss the benefits of PHP/IOP services as part of their continuum of care treatment and ensure proper referral is made.
  • Have a clear understanding of the levels of service and tracks available in the outpatient programs.
  • Understand the inpatient workflow process as it pertains to case management and discharge planning.
  • Collect data-points specific to the step-down process and entry of this data into MIDAS.
  • Review and run reports out of MIDAS in an effort to identify trends providing weekly updates to hospital leadership.
  • Work closely and collaborate with social services, business office, utilization review and outpatient departments.
  • Demonstrates ability to maintain therapeutic relationships, which do not allow self-disclosure of personal/social information to patients and adheres to the Professional Boundary Policy.
  • Facilitate outpatient groups based on current patient needs and overall programmatic goals.
  • Other duties as assigned.

Qualifications

Education and Experience Requirements:

  • High School Graduate or equivalent.
  • Prior clinical experience is preferred.
