Job Title : Licensed Practical Nurse (LPN General)

Location : Grafton, OH 44044

Duration : 6 Months to long term

Shift: 2:00 PM - 10:00 PM

Onsite: Yes

Role & Responsibilities:

  • Perform skilled practical nursing procedures and treatments (e.g., obtaining vital statistics, applying oxygen, sterile dressing, inserting catheters, administering medications, wound treatments, enemas, administering enteral feedings, etc.).
  • Observe, document, and report patient conditions, taking appropriate actions as necessary.
  • Provide thorough and knowledgeable reports of patient needs to the RN/Physician.
  • Monitor patients during and after procedures and assist RN and Physician as needed.
  • Respond to emergencies, provide first aid, perform CPR, and use AED in emergency situations.
  • Maintain medical accountability records and prepare related reports/forms (e.g., patient charts, medication administration records, unusual incident forms, and more).
  • Verify and implement Physician's orders, complete controlled drug administration/tracking records, and maintain infection control documentation.
  • Maintain supplies and keep the work area appropriately stocked and organized (ordering, inventorying, delivering, and obtaining supplies).
  • Communicate with pharmacy to maintain medication stock.
  • Assist with clerical tasks such as answering phones, filing, copying, and faxing.
  • Provide general nursing care and assist in the orientation and training of lower-level direct staff in unskilled nursing care (e.g., feeding, dressing, bathing, transferring, and repositioning patients).
  • Attend medical appointments as needed and participate in meetings as required.
  • Respond to patient alarms and assist in maintaining appropriate patient supervision levels.
  • Confer with the Nursing Supervisor or Director of Nursing about nursing needs and personnel-related matters.


Education Required:


  • Associate's Degree
  • Associate's Degree in Nursing
  • Experience: At least 6 months of relevant nursing experience is preferred.
  • License: Valid Ohio Medical License
  • Certifications: CPR certification, COVID vaccine card or exemption letter
