UCLA Health is seeking a dynamic Community Engagement and Partnerships (CEP) Manager to play a pivotal role in advancing our Social Drivers of Health (SDOH) Program. This position is responsible for managing and supporting the day-to-day operations of community engagement initiatives and strategic partnerships, with a focus on building and sustaining a trusted network of Community-Based Organizations and social care resources to better support UCLA Health patients. Reporting to the Assistant Director of the UCLA Health SDOH Program, the CEP Manager collaborates with internal teams and health system leaders to design and implement innovative strategies that expand social care resources, strengthen intra-organizational and inter-organizational community partnerships, and enhance stakeholder engagement. Key Responsibilities:
- Oversee daily administrative operations and execute tasks in support of the SDOH community engagement efforts
- Develop and manage strategic partnerships with Community-Based Organizations and other intraorganizational departments that provide social care resources
- Lead cross-functional initiatives and collaborate with internal teams to support program goals, enhance and deliver trainings
- Coordinate and implement community engagement activities and events
- Develop and promote best practices for partnership building and stakeholder engagement
- Create and maintain engagement tools, outreach materials, and curated resource lists
- Facilitate training and onboarding for internal staff and external partners on social care referral platforms and relevant interoperability tools
- Monitor and promote effective use of the referral platforms across all users
- Support the processing and execution of partner agreements, ensuring alignment with program objectives and organizational policies
- Represent the SDOH Program at partner sites, collaborative meetings, and relevant conferences
- Promote program visibility and identify opportunities for growth, collaboration, and training
Note: This is a flexible hybrid position with anticipated on-site days.
Salary Range: $76,200 - $158,800 Annually
Qualifications
- Bachelor's degree or a combination of equivalent education/experience/training
- Minimum three years of experience in community engagement, health promotion, and/or establishing and maintaining cross-organizational partnerships is required
- Demonstrated understanding of health-related social needs, population health, and related payment models or frameworks is strongly preferred
- Experience in developing and implementing quality improvement initiatives and methodologies, and organizational development is highly desirable
- Strong analytical and problem-solving skills, with the ability to analyze and synthesize information, identify patterns, and formulate actionable conclusions
- Proficiency in managing competing demands and priorities in a dynamic and collaborative work environment
- Excellent verbal, written, and interpersonal communication skills, including conveying complex information diplomatically to diverse audiences, such as physicians, administrators, leadership, community members, and community stakeholders
- Advanced proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Teams, Outlook) and Adobe tools
- Familiarity with project management methodology, platforms and ability to document effectively within these systems
- Proven ability to develop and maintain positive working relationships with a wide range of stakeholders, including patients, community-based organization staff, clinical advisors, community health workers, and internal teams
- Exceptional leadership and demonstrated independent decision-making ability, with a strong commitment to quality, inclusive excellence, and learning health system principles
- Ability to travel to community-based organizations, UCLA Health sites, relevant consortium meetings and conferences to facilitate engagement, training, and relationship building
- Demonstrated ability to follow through on assignments independently while integrating feedback and working effectively as part of a team
