LPN Dementia Services Coordinator (Webster Campus)

The primary purpose of your job is to supervise resident care, implementation of case management, supervise medication assistance, and overall care provided by LPNs, Resident Care Aides and other employees on the Memory Care Unit (Rainier Grove), and to assure that the resident care meets / exceeds the requirements as specified by the NYSDOH.

Job Description:



  • Ensures that staff provide residents with quality personal care services, ensuring the quality of life and the highest level of functioning for each person.
  • Responsible for overall maintenance of resident charts and the organization of the medication room
  • Assists with completion of related documentation for admission, transfer or discharge of Memory Care residents.
  • Assist Case Manager with overall development, implementation and review of Individualized Service plans of care for Memory Care residents.
  • Responsible for the coordination of medical, nursing, psychosocial and other services for those who live in Rainier Grove
  • Provides supervision and/or assistance with personal care needs, assistance with toileting, bathing, dressing, grooming and mobility programs needed to promote quality of life and independent functioning of each resident.
  • Supervises resident medication administration and assists in performance of prescribed treatments.
  • Keeps accurate inventory of narcotics and completes documentation for the shift worked.
  • Observes, reports and documents change in resident's health and/or behavior
  • Contacts responsible party on behalf of resident to report changes in conditions and/or falls.
  • Reports resident incidents to the Assisted Living Administrator and completes incident reports.
  • Provides direction, education, growth and development to nursing staff and Resident Care Aides in the Memory Care Unit to assure effective and appropriate delivery of services to residents including:
  • Participation in the selection and hiring of staff
  • Orientation of new staff
  • Ongoing evaluation of all health-related interventions
  • Developing plans for ongoing in-service education for staff


Candidate Requirements:



  • Current Licensed Practical Nurse (LPN) in good standing.
  • Prior supervisory experience
  • Knowledge of functions and responsibilities in provision of services to geriatric residents.
  • Experience with electronic medical records preferred

  • Must attend all mandatory in-service training, and meet continuing education requirements.
