Director of Clinical Services (NYS RN License required), Westchester, NY

We are looking for a Director of Clinical Services to work for one of our external clients, a non-profit organization that provides home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area. The office is located in Westchester, NY.

Must have a NYS RN License. Additionally, an ideal candidate would have a nursing degree, and completed NYSED-approved infection control coursework; minimum of two to three (2 - 3) years of Clinical Staff Supervisory experience, as well as experience with CHHA, preferred.

This is a permanent, full-time job. Schedule: Monday - Friday 8:30 AM - 5:30 PM, 40 hours per week.

In this role you will be responsible for patient services provided by the LHCSA and coordination of the multi-disciplinary integrated care in the community, Day Health Center, and Diagnostic & Treatment Center.

Responsibilities include, but not limited to:



  • Oversees and supports site-based managers and clinical staff, including DTC, GCC, rehab, nutrition, social work, and home services.
  • Balances and allocates resources to ensure proper team assignments and coverage.
  • Develops and executes personalized care plans to improve participant satisfaction and outcomes.
  • Oversees service provision impacting participant care and implements training to advance team members.
  • Collects operational metrics to ensure quality care at optimal cost.
  • Participates in quality assurance and regulatory compliance activities.
  • Serves as LHCSA DPS and Nursing Director for D&T; supervises DTC RN.
  • Provides administrative on-call support during non-operational hours.
  • Collaborates with clinical development departments to improve care delivery processes.
  • Manages enrollment and disenrollment in the HCS System.
  • Acts within professional authority and meets PACE competencies.
  • Ensures documentation accuracy and investigates incidents and grievances.
  • Participates in special projects and assignments as needed.
  • Evaluates, supports, and holds staff accountable while managing disciplinary actions.
  • Develops professional development plans and resolves employee issues.
  • Participates in the departmental budgeting process and manages expenditures.
