Overview

Under the general supervision of the Director of Oncology this role will strategically identify and coordinate opportunities within the community to offer disease prevention and wellness initiatives founded on evidenced based national guidelines to effectively raise the health of the community through cutting edge programs. This position will also provide management and oversight of the Parish Nurse Program through which a dynamic, innovative, educational, healing, and non-invasive health care ministry can take place through a partnership between UnityPoint Trinity and area congregations as well as other community sites.


Responsibilities

Prevention, Wellness and Community Outreach

  • Develops an annual wellness and prevention plan related to disease prevention, screening and educational programs and ensures that the provided programs reflect the community-defined needs.
  • Interfaces with media to educate community on UPH wellness initiatives and offerings open to the public.
  • Ensures that the prevention and screening activities follow nationally accepted, evidence-based prevention and wellness guidelines and interventions.
  • Research national and regional programs for new ideas, events, and best practices.
  • Develops/maintains and grows relationships in conjunction with partner community and national organizations.
  • Coordination with marketing on community outreach activities and internal UPH activities
  • Organizes and coordinates program events.

Parish Nurse Program

  • Manages day to day functions, processes, identifies variations, and resolves issues related to the volunteers and employees of the parish nurse program. Develops, updates, and communicates mission statement, philosophy, goals, practice standards, policies and procedures.
  • Oversees and coordinates enrollment of staff in the Parish Nurse Foundations Course.
  • Issues and renews agreements with Trinity Health Foundation and affiliate churches on employment of staff.
  • Oversees program development and serves as a liaison between churches and UnityPoint Trinity.
  • Identifies and recommends opportunities for improvement in revenue production and cost reduction.
  • Engages and communicates with staff providing updates, expectations, hospital mission and vision and ways to improve quality of care to our patients and families.

Monitoring and Reporting

  • Monitor the effectiveness of prevention, screening and outreach activities as directed by the organization.
  • Establish mechanisms to ensure follow-up of all positive findings identified through screening activities.
  • Evaluate the effectiveness of access and referral processes of screening and early detection activities to ensure appropriate referrals are made.
  • Create a community outreach activity summary report that outlines the activities provided, the results of outreach programs, and follow-up. The report should specifically focus on prevention and screening activities and must contain, at a minimum, the following information: identified areas of community need, specific community outreach activities performed, and summary of effectiveness of each activity.
  • Continuously measures and analyzes partnership programs for success and further development.
  • Reports outcomes and participates in quality improvement activities.
  • Monitors the expenditures for projects/events to assure alignment with the budget.

Qualifications

Education:

  • Three or more years' experience in community outreach, marketing or health administration.
  • Bachelor's Degree in Health Administration, public health or nursing is preferred; or an equivalent combination of education and experience from which comparable knowledge and skill can be acquired is necessary.

Experience:

  • Any combination of training and experience, which would provide the required knowledge and abilities equivalent to a bachelor's degree with a major in public health, education, social or behavioral science, or marketing.
  • Clinical knowledge and management experience preferred

License(s)/Certification(s):

  • Valid driver's license when driving any vehicle for work-related reasons
