Certified Aide - Home Health (part time, 20 hrs)

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care and emotional comfort services as necessary to meet the patient's needs.The Home Care aide travels to patient homes and provides in-home care to assigned patients daily The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performed.

Principal Responsibilities



  • Providing hands-on personal care including:

    • Baths
    • Back rubs
    • Oral hygiene
    • Shampoos

  • Changing bed linen
  • Assisting patients with dressing and undressing
  • Skin care to prevent breakdown.
  • Assisting the patient with toileting activities
  • Keeping patient's living area clean and orderly, as appropriate
  • Planning and preparing nutritious meals.
  • Assisting in feeding the patient, if necessary.
  • Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
  • Assisting in ambulation and exercise according to the plan of care.
  • Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
  • Performing simple procedures as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).
  • Assisting patient in the self-administration of medication.
  • Doing patient's laundry, as appropriate.
  • Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
  • Reporting on patient's condition and significant changes to the assigned nurse.
  • Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct.
  • Comply with the Springfield Clinic incident reporting policy and procedures.
  • Adhere to all OSHA and Springfield Clinic training & accomplishments as required per policy.
  • Provide excellent customer service and adhere to Springfield Clinic's Code of Conduct and Ethics Standards.
  • Perform other job duties as assigned.


Licenses/Certificates



  • Registered in IL as a Certified Nursing Assistant
  • CPR certification per American Heart Association guideline required within 30 days of hire.
  • Reliable means of transportation, valid IL driver's license and vehicle liability.


Knowledge, Skills and Abilities



  • Ability to read and follow written instructions and document care given.
  • Self-directing with the ability to work with little direct supervision.


Working Environment



  • This job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and car.
  • Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids and infectious diseases
