Overview $5,000 sign-on bonus for external candidates UnityPoint at Home Social Worker - LISW or LMSW Central Iowa Schedule: 40 hours a week, Monday-Friday, 8am- 5pm Full-time with benefits Responsibilities Patient Care Performs comprehensive psychosocial and behavioral assessments, treatment plans and appropriate interventions and therapy for behavioral health illnesses and concerns. Care Coordination Conducts assessment, diagnosis and treatment of the social and emotional factors related to the patients illness, need for care, response and adjustment to illness and care and completes appropriate patient-centered plan of care and interventions according to the scope of Hospice. Provides information and referral services to appropriate individuals and agencies as needs identified. Clinical Excellence Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patient. Participates in the development of the plan of care and attends interdisciplinary team meetings. Qualifications Education: Master's degree in social work with accreditation by the council on Social Work Education Experience: One year clinical experience in health care. Previous hospice and home care experience, is preferred. License(s)/Certification(s): LISW: Licensed Independent Social Worker/Licensed Clinical Social Worker - licensure in good standing as required by applicable law. OR LMSW: State Licensure per State Code. Valid driver's license when driving any vehicle for work-related reasons. Mandatory Reporter: Child & Dependent Adult Abuse Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities. CPR Certification Knowledge/Skills/Abilities: Strong interpersonal skills and customer service skills. Strong verbal and written communication skills. Ability to take initiative and exercise independent judgment, decision making and problem solving expertise. Ability to work as a collaborative team member.
