Pharmacist, Nights, 7/7, New Brunswick, NJ

Job Overview:

The Pharmacist fulfills daily operational responsibilities which assure the safe and effective use of medications in our patients. The Pharmacist adheres to contemporary standards of practice in fulfilling these responsibilities. The Pharmacist fulfills these requirements working a 7 night on, 7 night off schedule. This position will work in a decentralized capacity out of the emergency department for the duration of shift but must also be amendable to working from Main Pharmacy, when necessitated by scheduling.

Qualifications:

Required:

• Five or six years
• Bachelor of Science of Doctor of Pharmacy degree from an ACPE accredited US Pharmacy school or Foreign pharmacy school with appropriate US credentials,
• with current New Jersey Board of Pharmacy license and registration.

Preferred:

• Advanced degree in a Pharmacy, business or science discipline,
• One year of an accredited pharmacy residency
• BCPS board certification

Essential Functions:

• Reviews physician orders for Appropriateness of drug, dose, frequency, and route of administration, Therapeutic duplication, Age appropriateness, Allergies or sensitivities, Current or potential impact as evidenced by laboratory values, Food drug and Drug drug interactions, Other complications
• Compares new orders with current patient specific information and resolves all drug related problems, in conference with prescribing physician when necessary,
• Provides pharmacy documentation of interventions,
• Communicates and confers with both nursing staff and patients in order to obtain relevant clinical information,
• To provide information to support the nurses responsibilities for administering medications, monitoring drug therapy, and providing patient education,
• When appropriate in order to obtain relevant clinical information,
• When possible, and when appropriated, educates patients regarding medications prescribed at discharge
